As a Remote Clinical Coordinator, you will play a pivotal role in ensuring the seamless flow of patient care by coordinating various clinical activities, managing schedules, and facilitating critical communication between patients, providers, and external stakeholders. This role is crucial for maintaining the high standards of quality and efficiency that UnityPoint Health is known for, even in a distributed work environment. You will be instrumental in supporting complex patient journeys, ensuring timely access to necessary services, and contributing to positive health outcomes across our patient population.

The ideal candidate will possess a strong clinical background, exceptional organizational skills, and a proven ability to manage multiple priorities in a fast-paced setting. Your day-to-day will involve a mix of administrative support, patient advocacy, and proactive problem-solving. You will utilize advanced communication technologies to connect effectively with team members and patients, ensuring that geographical distance does not impede the quality or promptness of care. This role requires a high degree of autonomy and self-discipline, coupled with a collaborative spirit, as you will be expected to work independently while contributing to broader team goals and initiatives. We are looking for someone who is not only proficient in clinical coordination but also passionate about leveraging technology to improve healthcare accessibility and enhance the patient experience. Your expertise will help us uphold our mission to improve the health of the people and communities we serve by providing the best outcome for every patient, every time.

In this vital remote role, you will be the central point of contact for a diverse caseload, meticulously managing patient records, scheduling appointments, coordinating diagnostic tests, and facilitating referrals to specialists. You will work closely with physicians, nurses, and and other allied health professionals to develop and implement individualized care plans, ensuring that all aspects of a patient’s treatment journey are aligned and executed efficiently. Beyond the logistical aspects, a significant part of your role will involve patient education and support, empowering individuals and their families with the information and resources they need to navigate their healthcare options. You will also be responsible for maintaining accurate and up-to-date documentation in our electronic health record system, adhering strictly to HIPAA regulations and organizational policies regarding patient privacy and data security. The ability to quickly adapt to new technologies and processes is essential, as UnityPoint Health is continuously innovating to enhance patient care and operational effectiveness. Your contributions will directly support our commitment to delivering accessible, coordinated, and compassionate care, helping us bridge geographical gaps and serve a broader population effectively. This position is perfect for a proactive individual who thrives on making a difference, values continuous learning, and is eager to contribute to a leading healthcare system dedicated to innovation and patient well-being.
