Key Responsibilities
- Conduct comprehensive reviews of clinical documentation in patient medical records to ensure accuracy, completeness, and specificity.
- Formulate and submit compliant queries to physicians and other healthcare providers for clarification and additional documentation, adhering to established guidelines.
- Educate physicians, residents, and other clinical staff on documentation best practices, coding requirements, and the impact of accurate documentation on quality outcomes and reimbursement.
- Collaborate with coding professionals to ensure alignment between clinical documentation and coded data.
- Monitor and analyze documentation trends, identifying areas for improvement and contributing to ongoing education initiatives.
- Stay current with changes in official coding guidelines (ICD-10-CM/PCS), regulatory requirements, and industry best practices for clinical documentation.
- Participate in interdisciplinary team meetings and quality improvement projects related to documentation excellence.
- Maintain strict confidentiality of patient information and adhere to all HIPAA regulations and organizational policies.
Required Skills
- Active Registered Nurse (RN) license or equivalent clinical licensure (e.g., MD, DO, PA, NP)
- Minimum of 2 years of experience in clinical documentation improvement (CDI) or a related healthcare field (e.g., case management, utilization review, coding)
- In-depth knowledge of medical terminology, anatomy, physiology, pathophysiology, and pharmacology
- Strong understanding of ICD-10-CM/PCS coding principles and guidelines
- Excellent written and verbal communication skills with the ability to effectively query physicians and educate staff
- Proficiency with Electronic Medical Record (EMR) systems and other healthcare IT platforms
- Exceptional attention to detail and analytical problem-solving abilities
- Ability to work independently and manage time effectively in a remote work environment
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) or higher degree
- Certified Clinical Documentation Specialist (CCDS) or Certified Documentation Improvement Practitioner (CDIP) certification
