Key Responsibilities
- Assist Care Coordinators in the creation, review, and updating of personalised care plans, ensuring they accurately reflect client needs and preferences.
- Maintain meticulous and confidential client records, including care plan documentation, assessments, and communication logs, in compliance with data protection regulations.
- Liaise effectively with clients, their families, and external healthcare professionals to gather essential information and ensure smooth care transitions.
- Provide administrative support to the care team, including scheduling, managing correspondence, and preparing reports.
- Monitor the implementation of care plans and flag any deviations or concerns to the relevant Care Coordinator for prompt action.
- Conduct regular welfare checks and feedback gathering from clients to ensure satisfaction and identify areas for improvement in care delivery.
- Contribute to team meetings, offering insights and suggestions to enhance service quality and client outcomes.
Required Skills
- Proven experience in an administrative or support role within a healthcare or social care setting.
- Exceptional organisational skills and attention to detail, with the ability to manage multiple tasks efficiently.
- Strong verbal and written communication skills, capable of empathetic and professional interaction.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and comfortable with digital record-keeping systems.
- Demonstrated ability to maintain confidentiality and adhere to professional ethics.

Preferred Qualifications
