Key Responsibilities
- Manage a caseload of clients, assessing their needs and developing individualized service plans.
- Coordinate and schedule appointments with various service providers, including medical, therapeutic, and social services.
- Act as a primary point of contact and advocate for clients, ensuring their voices are heard and needs are met.
- Monitor client progress, evaluate the effectiveness of service plans, and make adjustments as necessary.
- Maintain accurate, up-to-date, and confidential client records and documentation.
- Liaise effectively with families, caregivers, healthcare professionals, community organizations, and other stakeholders.
- Facilitate communication and collaboration among all parties involved in a client's care plan.
- Provide information, resources, and referrals to clients and their families.
- Prepare comprehensive reports on client status, service utilization, and outcomes.
- Ensure compliance with all relevant policies, procedures, and regulatory requirements.
- Participate in team meetings, case conferences, and professional development opportunities.
Required Skills
- Proven experience in case management or a similar coordination role.
