Key Responsibilities

  • Assist in the development and implementation of personalised care plans for clients.
  • Coordinate daily schedules for care professionals and clients, ensuring optimal service delivery and continuity of care.
  • Communicate regularly and empathetically with clients and their families, addressing queries, providing updates, and building strong relationships.
  • Maintain accurate, confidential, and up-to-date client records in compliance with data protection regulations and company policies.
  • Liaise effectively with healthcare providers, social workers, and other external agencies to ensure holistic client support and integrated care.
  • Provide comprehensive administrative support to the care coordination team, including report generation, documentation, and managing correspondence.
  • Participate actively in team meetings, supervision sessions, and ongoing training to foster continuous professional development.
  • Monitor and review the effectiveness of care services, making recommendations for adjustments and improvements as needed.
