Key Responsibilities

  • Actively engage with diverse community members, conducting initial needs assessments and effectively connecting them with appropriate health, social care, and welfare services.
  • Assist in the meticulous planning, dynamic promotion, and impactful delivery of various health promotion activities, educational workshops, and outreach programs.
  • Provide compassionate, practical support and ongoing encouragement to individuals, guiding them in adopting and maintaining healthier behaviors and lifestyle choices.
  • Maintain accurate, confidential, and comprehensive records of all client interactions, service referrals, and follow-up activities using designated systems.
  • Collaborate seamlessly and effectively with a wide range of local health professionals, community groups, charitable organizations, and internal Career.zycto teams.
  • Serve as a trusted and accessible point of contact for residents, offering empathetic listening, vital information, and precise signposting to relevant support networks.
  • Actively participate in and fully commit to all provided comprehensive training and continuous professional development programs, embracing a growth mindset.
  • Contribute to the identification of community health needs and assist in the development of targeted initiatives to address them.
