Key Responsibilities

  • Conduct initial client assessments to identify individual care needs and preferences.
  • Develop, implement, and regularly review personalised care plans in collaboration with clients, families, and healthcare professionals.
  • Coordinate and manage care schedules for a team of community care workers, ensuring optimal client-carer matching and service delivery.
  • Act as the primary point of contact for clients, families, and care staff, addressing concerns and providing support.
  • Monitor the quality of care provided, ensuring adherence to CQC standards and company policies.
  • Recruit, train, and supervise care staff, fostering a positive and professional working environment.
  • Maintain accurate and up-to-date client records and administrative documentation.
  • Participate in on-call rotations as required to provide out-of-hours support.
  • Facilitate effective communication between clients, carers, and external agencies.
  • Promote the independence and well-being of clients through tailored care solutions.

Required Skills

  • Proven experience in care coordination or a similar role within the social care sector (minimum 2 years).
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
  • Strong organisational and time management abilities, capable of managing multiple priorities.
  • Proficiency in using care management software and standard office applications.
  • Ability to work independently and as part of a team, demonstrating initiative and problem-solving skills.
  • Full UK driving license and access to a reliable vehicle for community visits.
  • Knowledge of relevant care standards and regulations (e.g., CQC, SSSC).

Preferred Qualifications

  • SVQ Level 3 in Health and Social Care or equivalent.
  • Experience with rostering and scheduling software specific to care services.
  • Familiarity with the local Edinburgh community care landscape.
  • Previous experience in a supervisory or leadership capacity within a care setting.
