Key Responsibilities

  • Assist in the initial assessment of clients' needs and contribute to the development of personalised care plans.
  • Coordinate and schedule various care services, ensuring timely and appropriate support for clients.
  • Act as a primary point of contact for clients, their families, and care providers, facilitating clear communication.
  • Maintain accurate and confidential client records, ensuring compliance with data protection regulations.
  • Monitor the effectiveness of care plans and contribute to reviews, making adjustments as necessary.
  • Advocate for clients' rights and preferences, ensuring their voices are heard in care decisions.
  • Liaise with external agencies and community resources to enhance client support networks.
  • Identify and report any safeguarding concerns or risks, following established procedures.
