Key Responsibilities
- Develop, review, and update individualised care plans in collaboration with clients, families, and healthcare professionals.
- Coordinate and schedule care services, ensuring efficient resource allocation and timely delivery.
- Act as the primary point of contact for clients and their families regarding care plans and services.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to meet evolving needs and client preferences.
- Maintain accurate, up-to-date, and confidential client records, adhering strictly to data protection guidelines and organisational policies.
- Facilitate clear and consistent communication between clients, care staff, external agencies, and other stakeholders.
- Provide emotional support and reassurance to clients and their families, fostering a trusting and respectful relationship.
- Participate actively in team meetings, supervision sessions, and ongoing training opportunities to enhance professional skills and knowledge.
- Ensure compliance with all relevant care standards, legislative requirements, and organisational policies and procedures.
