Key Responsibilities
- Assist in the development, implementation, and review of individual support plans for service users.
- Coordinate appointments and activities for individuals with learning disabilities.
- Maintain accurate and confidential records, ensuring compliance with data protection policies.
- Liaise with families, carers, and external professionals (e.g., social workers, healthcare providers) under supervision.
- Provide administrative support to the coordination team, including scheduling, correspondence, and report generation.
- Participate in team meetings and training sessions to enhance knowledge and skills.
- Advocate for the needs and wishes of service users, promoting their independence and inclusion.
- Help to organise and facilitate group activities and community engagement initiatives.
- Monitor and report on service user progress and any concerns to senior staff.
- Ensure all activities comply with organisational policies, procedures, and relevant legislation.
Required Skills
- Excellent communication and interpersonal skills, both written and verbal.
- Strong organisational and time management abilities.
- Proficiency in basic computer applications (e.g., Microsoft Office Suite).
