Key Responsibilities
- Conduct comprehensive initial assessments and regular reviews of client care needs.
- Develop, implement, and monitor individualised care plans tailored to client requirements.
- Coordinate daily care schedules, liaising with carers, clients, and their families.
- Act as the primary point of contact for a caseload of clients, addressing concerns and providing support.
- Ensure compliance with all relevant care standards, regulations, and company policies.
- Lead, mentor, and support a team of care professionals, fostering a positive work environment.
- Manage and resolve challenging situations or complaints with professionalism and empathy.
- Maintain accurate and up-to-date client records and documentation.
- Collaborate with external healthcare professionals and community services.
- Participate in on-call rotations as required to provide out-of-hours support.
Required Skills
- Proven experience in elderly care coordination or a similar supervisory role.
- Excellent communication and interpersonal skills, both verbal and written.
- Strong organisational and time management abilities.
- Demonstrated empathy, compassion, and a person-centred approach.
- Proficiency in using care management software and general office applications.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Sound knowledge of care regulations and best practices.
Preferred Qualifications
- NVQ Level 3 or higher in Health and Social Care.
- Full UK Driving License and access to a vehicle.
- Experience with CQC (Care Quality Commission) compliance.
- Previous experience with budget management or resource allocation.
