As a Care Plan Lead, you will be instrumental in coordinating and overseeing the development, implementation, and review of personalised care plans for our clients. You’ll work closely with individuals, their families, and our dedicated care team to ensure that every care plan is tailored to meet unique needs, promoting independence, dignity, and overall well-being. Key Responsibilities

  • Develop, implement, and regularly review individualised care plans in collaboration with clients, families, and healthcare professionals.
  • Lead and mentor a team of care staff, providing guidance, support, and oversight to ensure the delivery of high-quality care.
  • Conduct initial assessments and ongoing evaluations of client needs, adapting care plans as circumstances change.
  • Ensure all care activities adhere to regulatory standards, company policies, and best practice guidelines.
  • Maintain accurate and up-to-date records of care plans, daily logs, and client progress.
  • Act as a primary point of contact for clients and their families regarding care plan queries and concerns.
  • Facilitate effective communication between care team members, clients, and external stakeholders.
  • Participate in continuous professional development and training initiatives to enhance leadership and care planning skills.

Required Skills

  • Minimum of 2 years of experience in a care-giving role (e.g., Support Worker, Care Assistant).
  • Strong communication and interpersonal skills, with the ability to build rapport and trust.
  • Excellent organisational abilities and attention to detail.
  • Demonstrated empathy, patience, and a person-centred approach to care.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Proficiency in basic computer applications for record keeping and communication.

Preferred Qualifications

  • NVQ Level 2 or 3 in Health and Social Care (or equivalent qualification).
  • Experience with care planning software or electronic record systems.
  • Previous experience in a supervisory or senior care role is an advantage, but not essential due to provided training.
