Key Responsibilities

  • Develop, implement, and monitor individualised care plans in collaboration with clients, families, and healthcare professionals.
  • Act as the primary point of contact for clients, their families, and caregivers, addressing inquiries and concerns promptly and professionally.
  • Effectively manage and optimise caregiver schedules, ensuring appropriate staffing levels and continuity of care.
  • Maintain accurate and confidential client records, care plans, and administrative documentation in compliance with data protection regulations.
  • Liaise with external agencies, medical practitioners, and community services to facilitate holistic support for clients.
  • Conduct regular virtual reviews and assessments to ensure care plans remain relevant and effective.
  • Provide guidance and support to caregivers, assisting with complex cases and ensuring adherence to best practices.
  • Identify and mitigate potential risks, escalating concerns to senior management when necessary.
  • Contribute to the continuous improvement of services and operational efficiency within the remote care team.

Required Skills

  • Proven experience (2+ years) in a care coordination or similar role within the health or social care sector.
  • Exceptional organisational and time management skills, with the ability to manage multiple priorities effectively.
  • Outstanding verbal and written communication skills, demonstrating empathy and clarity.
  • Proficiency in using IT systems, including care management software, CRM tools, and Microsoft Office Suite.
  • Strong problem-solving abilities and a proactive approach to challenges.
  • Ability to work independently and manage workload effectively in a remote setting.
  • A deep understanding of care regulations, safeguarding principles, and best practices.

Preferred Qualifications

  • NVQ Level 3 in Health and Social Care or equivalent qualification.
  • Experience with specific care planning and scheduling software (e.g., CarePlanner, CareLineLive).
  • Previous experience working in a remote or home-based role.
  • Knowledge of local community resources and support networks in the Hale, Farnham area.
