Key Responsibilities
- Manage a complex caseload of clients, developing and implementing individualized, person-centered care plans.
- Serve as the primary advocate for clients, navigating healthcare systems and coordinating services with various providers.
- Collaborate effectively with a multidisciplinary team (physicians, nurses, social workers, therapists) to ensure integrated care.
- Conduct thorough client assessments to identify needs, strengths, and goals, regularly updating care plans.
- Educate clients and families on health conditions, treatment options, and available community resources.
- Monitor client progress and outcomes, making timely adjustments to care plans as necessary.
- Ensure strict adherence to all healthcare regulations, ethical guidelines, and company policies.
- Mentor and provide guidance to junior care coordination staff, fostering a collaborative learning environment.
- Participate in and contribute to continuous quality improvement initiatives and program development.
- Maintain accurate and confidential client records using designated software systems.
Required Skills
- Minimum of 4 years of experience in care coordination, case management, or a related healthcare field.
- Exceptional communication skills, both verbal and written, in English and French.

- Demonstrated empathy, compassion, and client advocacy skills.
