Key Responsibilities
- Conduct comprehensive psychosocial assessments to determine client needs and strengths.
- Develop and implement individualized care plans, setting measurable goals and objectives.
- Provide crisis intervention, counseling, and support to individuals and families in need.
- Advocate for clients' rights and access to resources, including housing, healthcare, employment, and legal aid.
- Maintain accurate and timely client records, documentation, and progress notes in compliance with regulatory standards.
- Collaborate with interdisciplinary teams, community organizations, and external agencies to coordinate services and referrals.
- Monitor client progress, evaluate effectiveness of interventions, and adjust care plans as necessary.
- Educate clients and families on available resources, coping strategies, and self-advocacy skills.
- Participate in team meetings, case conferences, and professional development activities.
Required Skills
- Master's degree in Social Work (MSW) from an accredited program.
- Current Indiana Social Worker license (LSW or LCSW preferred, or actively pursuing).
- Minimum of 2 years of post-graduate experience in a social work setting.
- Proficiency in conducting psychosocial assessments and developing care plans.
- Demonstrated ability to provide crisis intervention and counseling.
- Strong understanding of community resources and social service systems.
- Excellent verbal and written communication skills.
- Proficiency in electronic health records (EHR) systems and Microsoft Office Suite.
- Ability to work effectively with diverse populations and backgrounds.
Preferred Qualifications
- Experience with specific populations such as mental health, substance abuse, child welfare, or geriatrics.
