Job Description

Career.zycto is proud to partner with a leading community services organization in Kensington Market, Toronto, to recruit a dedicated and dynamic Full-Time Case Manager. As a Case Manager, you will serve as a crucial link, empowering clients to access essential resources, navigate social systems, and achieve their personal goals. Your day will involve conducting comprehensive assessments of client needs and strengths, developing individualized care plans that are client-centered and culturally sensitive, and facilitating access to a wide array of services including housing, mental health support, addiction services, employment assistance, legal aid, and educational opportunities. You will build strong, trusting relationships with clients, providing ongoing support, compassionate guidance, crisis intervention when necessary, and robust advocacy to ensure their voices are heard and their diverse needs are met with dignity and respect.

This position demands exceptional interpersonal skills, as you will collaborate extensively with an interdisciplinary team, including social workers, therapists, medical professionals, and support staff. You will also foster robust partnerships with external agencies, community organizations, governmental bodies, and healthcare providers to create a comprehensive network of support for your clients. You’ll be instrumental in coordinating services, monitoring client progress against established goals, and adapting strategies to evolving needs and circumstances. Key Responsibilities

  • Conduct comprehensive client needs assessments and develop individualized service plans.
  • Provide direct client support, advocacy, and crisis intervention as required.
  • Facilitate referrals and connect clients to appropriate community resources (housing, mental health, employment, etc.).
  • Collaborate effectively with interdisciplinary teams and external agencies to coordinate care.
  • Monitor client progress, regularly review service plans, and adjust interventions as needed.
  • Maintain accurate and timely client documentation in compliance with organizational standards.
  • Advocate for clients' rights and access to services within complex systems.
  • Participate in team meetings, case conferences, and professional development activities.

Preferred Qualifications

  • Master's degree in Social Work, Psychology, or a related field.
  • Proficiency in a second language relevant to Toronto's diverse population (e.g., Spanish, Mandarin, Cantonese, Portuguese).
  • Experience working with specific vulnerable populations (e.g., newcomers, youth at risk, individuals experiencing homelessness).
  • Certification in First Aid/CPR.
