Job Description
We are seeking a highly skilled and compassionate Full-Time Nurse Practitioner to join our dynamic healthcare team in the vibrant Riverside neighbourhood of Toronto. Key Responsibilities
- Conduct comprehensive health assessments, including physical examinations and health histories.
- Diagnose and treat common acute and chronic illnesses and injuries.
- Order and interpret diagnostic tests (e.g., lab work, X-rays).
- Prescribe medications and other therapies in accordance with provincial regulations.
- Develop and implement individualized care plans, collaborating with patients, families, and other healthcare professionals.
- Provide health education, counseling, and disease prevention strategies.
- Manage chronic disease conditions, including diabetes, hypertension, and asthma.
- Perform minor procedures within scope of practice.
- Maintain accurate and detailed patient records.
- Participate in quality improvement initiatives and continuing education.
- Advocate for patient needs and facilitate access to community resources.
Required Skills
- Current registration as a Nurse Practitioner (NP) with the College of Nurses of Ontario (CNO).
- Master's degree in Nursing (MN) or equivalent Nurse Practitioner program.
- Minimum 3 years of experience as a registered Nurse Practitioner, preferably in a primary care setting.
- Exceptional clinical assessment, diagnostic, and treatment skills.
- Proficiency in electronic medical records (EMR) systems.
- Strong communication, interpersonal, and patient education abilities.
- Ability to work autonomously and collaboratively within an interdisciplinary team.
- Demonstrated commitment to patient-centered care and cultural sensitivity.
Preferred Qualifications
- Experience with specific EMR systems used in Ontario (e.g., Telus Health PS Suite, Accuro, OSCAR).
- Bilingualism (English and French or other relevant languages for the Toronto population).
- Advanced Cardiac Life Support (ACLS) certification.
- Experience in community health or with vulnerable populations.
