Career.zycto is seeking a dedicated Full-Time Gerontologist to join our dynamic and supportive team in High Park, Toronto. Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical, cognitive, functional, and psychosocial evaluations.
- Diagnose and manage acute and chronic medical conditions common in older adults, such as dementia, frailty, falls, and polypharmacy.
- Develop and implement individualized care plans, collaborating with an interdisciplinary team to optimize patient outcomes.
- Provide expert consultation and guidance to primary care physicians and other healthcare professionals on geriatric issues.
- Educate patients and their families on disease management, preventive health, and healthy lifestyle choices for aging.
- Participate in case conferences, grand rounds, and quality improvement initiatives.
- Maintain accurate and timely electronic medical records in compliance with regulatory standards.
- Contribute to the development and implementation of evidence-based practices in geriatric medicine.
- Advocate for the unique needs of older adults within the healthcare system and community.
Required Skills
- Medical Doctor (MD) degree from an accredited institution.
- Specialty certification in Geriatric Medicine (FRCPC or equivalent).
- Valid medical license to practice in Ontario (CPSO registration).
- Minimum 3 years of clinical experience post-fellowship in Geriatric Medicine.
- Demonstrated expertise in managing complex geriatric syndromes and polypharmacy.
- Excellent diagnostic, clinical reasoning, and problem-solving skills.
- Strong interpersonal, communication, and empathy skills.
- Proficiency in electronic medical record (EMR) systems.
- Ability to work effectively in a multidisciplinary team environment.
Preferred Qualifications
- Experience with community-based geriatric care programs.
- Bilingualism (English and other commonly spoken languages in Toronto).
- Interest or experience in clinical research or academic contributions.
