This pivotal full-time role is designed for an empathetic and proactive individual who thrives on connecting with people, understanding their unique needs, and facilitating access to essential support services.
As a Community Outreach Support Worker, you will be on the front lines, engaging directly with community members who may be experiencing vulnerability, disadvantage, or isolation. This role requires a high degree of cultural sensitivity, excellent communication skills, and the ability to work autonomously while also collaborating effectively with a diverse team and external agencies.
You will be responsible for developing and maintaining strong relationships with local service providers, community groups, and key stakeholders to ensure a comprehensive network of support is accessible to our clients. A typical day might involve street outreach, home visits, attending community events, assisting clients with paperwork, advocating on their behalf, and providing crucial emotional support. Documentation and reporting are also key components, ensuring that client progress is tracked, and program outcomes are effectively measured.
