As a Community Nurse, you will be an integral part of our patient’s journey, delivering comprehensive care that extends beyond hospital walls. Your daily tasks will involve visiting patients in their homes, assessing their health needs, developing and implementing personalised care plans, and evaluating the effectiveness of interventions. This role demands a proactive approach to healthcare, requiring you to work autonomously while also collaborating effectively with multidisciplinary teams, including GPs, social workers, physiotherapists, and occupational therapists, to ensure holistic patient support.
The scope of practice for this role is diverse and challenging, encompassing everything from wound care management, medication administration, and chronic disease management to palliative care and health promotion. You will play a crucial role in educating patients and their families about their conditions, treatment plans, and self-management techniques, fostering an environment of understanding and empowerment. Key Responsibilities
- Conduct comprehensive nursing assessments for patients in their homes.
- Develop, implement, and evaluate individualised care plans.
- Administer medications and perform clinical procedures (e.g., wound care, catheter management).
- Monitor patient progress, recognising and responding to changes in health status.
- Provide health education and support to patients and their families.
- Liaise and collaborate with multidisciplinary healthcare teams.
- Maintain accurate and detailed patient records in line with professional standards.
- Identify and escalate safeguarding concerns appropriately.
- Manage a defined caseload efficiently and prioritise visits based on patient need.
- Adhere to all relevant policies, procedures, and professional codes of conduct.
Required Skills
- Current NMC registration (Registered General Nurse – RGN).
- Proven experience working as a Community Nurse or in a similar primary care setting.
- Demonstrable clinical skills including wound care, venepuncture, and medication administration.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and manage a caseload effectively.
- Strong problem-solving and decision-making abilities.
- Valid UK Driving Licence and access to a car for work purposes.
- Proficiency in electronic patient record systems.
Preferred Qualifications
- Community Specialist Practitioner qualification (e.g., District Nurse, Health Visitor).
- Experience in palliative care or chronic disease management.
- Advanced wound care certification.
- Knowledge of local Edinburgh healthcare pathways and resources.
