Key Responsibilities
- Provide one-on-one advocacy and support to clients, ensuring their rights and preferences are respected.
- Interpret complex medical information and bureaucratic procedures for clients and their families.
- Liaise with healthcare providers, social workers, legal aid, and other support services on behalf of clients.
- Develop and implement individualized advocacy plans based on client needs and goals.
- Maintain accurate and confidential client records, documenting all interactions and outcomes.
- Educate clients about available resources and support systems within the community and hospital.
- Assist clients in navigating appointments, understanding forms, and communicating effectively with professionals.
- Identify systemic issues and contribute to improvements in service delivery and patient experience.
- Attend relevant meetings, training sessions, and workshops to stay informed on best practices and regulations.
