Key Responsibilities

  • Monitor the effectiveness of care plans, making necessary adjustments to ensure client needs are continually met.
  • Act as a central point of contact for clients and their families regarding care planning and coordination.
  • Provide guidance and support to care staff on the implementation of care plans and best practice.
  • Ensure all care plans comply with relevant regulations, company policies, and quality standards.
  • Maintain accurate and up-to-date records of care plans, reviews, and client progress.
  • Facilitate regular meetings with care teams to discuss client progress and address any challenges.
  • Advocate for clients' rights and preferences, ensuring their voices are heard in care decisions.
  • Participate in ongoing training and professional development to enhance care planning expertise.
