Job Description

Career.zycto is seeking a dedicated and compassionate Community Care Lead to join our vibrant team in Woodthorpe, York. Key Responsibilities

  • Lead, supervise, and mentor a team of community care professionals.
  • Conduct comprehensive initial assessments and develop person-centred care plans.
  • Coordinate and monitor the delivery of high-quality care services.
  • Ensure compliance with CQC regulations, company policies, and safeguarding procedures.
  • Manage care worker rotas and ensure appropriate staffing levels.
  • Liaise effectively with service users, families, and external healthcare professionals.
  • Provide guidance and support to care staff, including regular supervisions and appraisals.
  • Manage and resolve client concerns and feedback promptly and effectively.
  • Contribute to continuous service improvement initiatives and best practice implementation.
  • Maintain accurate and up-to-date care records and documentation.
  • Participate in on-call duties as required by the flexible schedule.

Required Skills

  • Proven experience in a leadership or supervisory role within the health or social care sector.
  • Excellent communication, interpersonal, and negotiation skills.
  • Strong organizational abilities with a keen eye for detail and problem-solving.
  • Ability to effectively lead, motivate, and develop a diverse team.
  • Proficiency in care planning, assessment, and risk management.
  • Sound knowledge of CQC standards and relevant care legislation.
  • Full UK driving license and access to a reliable vehicle.
  • Competency in using IT systems for record-keeping and communication.

Preferred Qualifications

  • NVQ Level 3 or 4 in Health & Social Care (or equivalent qualification).
  • Experience working with specific client groups (e.g., dementia, learning disabilities, palliative care).
  • First Aid certification.
  • Experience in rota management software.
