Key Responsibilities

  • Lead, motivate, and manage a team of community care workers, ensuring high performance and professional development.
  • Oversee the planning, implementation, and review of individualised care packages in line with CQC standards and best practices.
  • Conduct regular supervisions, appraisals, and spot checks to ensure the quality and compliance of care delivery.
  • Act as a primary point of contact for service users, their families, and external healthcare professionals regarding care needs and concerns.
  • Manage staff rotas and allocate care assignments efficiently, adapting to service user needs and team availability.
  • Conduct initial assessments and regular reviews of service users' needs, ensuring care plans are accurate and up-to-date.
  • Participate in on-call duties on a rotational basis to provide out-of-hours support and guidance.
  • Contribute to the continuous improvement of care services by identifying training needs and implementing new initiatives.

Required Skills

  • Minimum 4 years of experience in the social care sector, with at least 2 years in a supervisory or leadership capacity.
  • Proficient understanding of CQC regulations and their practical application in community care settings.
  • Demonstrable leadership and team management skills, with a proven ability to motivate and develop staff.
  • Excellent communication, interpersonal, and organisational abilities.
  • Full UK driving licence and access to a vehicle for community travel.
  • Competency in using care management software and general IT applications.

Preferred Qualifications

  • NVQ Level 4 or 5 in Health and Social Care, or equivalent qualification.
  • Experience with electronic care planning and rostering systems.
  • Familiarity with the Hackney local authority and community resources.
