Key Responsibilities
- Conduct community outreach and education on health topics such as hygiene, nutrition, maternal and child health, and disease prevention (e.g., malaria, HIV/AIDS).
- Provide direct support to community members in accessing healthcare services, including appointment scheduling, transportation coordination, and navigating referral systems.
- Assist individuals and families in understanding medical instructions, prescriptions, and health management plans.
- Identify and assess community health needs and resources, reporting findings to the healthcare team.
- Organize and facilitate health workshops, screening events, and support groups within the community.
- Build and maintain trusting relationships with community members, local leaders, and healthcare providers.
- Maintain accurate and confidential records of client interactions, services provided, and health outcomes.
- Advocate for community health needs and serve as a liaison between the community and healthcare systems.
