This vital full-time role will involve working directly within the vibrant Ginger Hill and wider Savanna-la-Mar communities, serving as a crucial, trusted link between residents and essential healthcare services.
As a Community Health Supporter, you will play a pivotal role in promoting health literacy, facilitating seamless access to care, and empowering community members to take proactive charge of their health. Your daily activities will include conducting proactive outreach, organizing engaging and informative health education sessions, and providing empathetic, one-on-one support to individuals and families navigating the often-complex healthcare system. Key Responsibilities
- Conduct regular and proactive outreach activities within the Ginger Hill and wider Savanna-la-Mar communities to identify prevalent health needs and engage residents effectively.
- Organize, facilitate, and lead engaging health education workshops and informational sessions on critical topics such as chronic disease management, maternal and child health, nutrition, hygiene, and preventative care.
- Provide compassionate one-on-one support and personalized guidance to individuals and families, helping them understand their health conditions, manage care plans, and navigate the local healthcare system.
- Assist community members in accessing necessary medical appointments, understanding medication regimes, and connecting with other essential social services as required.
- Serve as a crucial liaison between community members and healthcare providers, ensuring clear, effective communication and advocating strongly for patient needs and preferences.
- Collect, maintain, and accurately record all data pertaining to community interactions, health education sessions, patient referrals, and program outcomes.
- Identify and actively work to address barriers to healthcare access, developing creative strategies to overcome them in close collaboration with the multidisciplinary healthcare team.
- Actively participate in community health fairs, screening events, and public health initiatives to promote the Medical Centre's services and broader public health messages.
- Regularly report on community feedback, emerging health issues, and the effectiveness of community health programs to the supervisory team and relevant stakeholders.
Required Skills
- High School Diploma or equivalent; an associate's degree or certification in a health-related field is a significant advantage.
- A minimum of 1-2 years of proven experience in community outreach, health education, social work, or a similar field.
- Exceptional interpersonal and communication skills (both verbal and written), with a demonstrated ability to build strong rapport and trust with diverse populations.
- Strong organizational skills, meticulous attention to detail, and the ability to manage multiple tasks and priorities effectively.
- Demonstrated empathy, patience, cultural sensitivity, and a non-judgmental approach to working with vulnerable individuals and communities.
- Deep familiarity with the Ginger Hill and wider Savanna-la-Mar community, including local resources, customs, and existing challenges.
- Proficiency in basic computer applications (e.g., Microsoft Office Suite, email) for record keeping, communication, and reporting.
- Ability to work effectively both independently with minimal supervision and collaboratively as a vital part of a multidisciplinary team.
- Possession of a valid driver's license and consistent access to reliable transportation for extensive community travel.
Preferred Qualifications
- Certification as a Community Health Worker (CHW) or a similar public health qualification.
- Comprehensive knowledge of local health policies, government-provided resources, and social support systems available in Jamaica.
- Prior experience working with electronic health records (EHR) systems or patient management software.
- Bilingual abilities (e.g., Patois and English) to better serve and engage diverse community members.
