Key Responsibilities
- Engage directly with community members to assess health needs and provide information on available resources.
- Assist individuals in navigating the healthcare system, including scheduling appointments and understanding medical information.
- Conduct outreach activities to promote health education workshops and preventive health screenings.
- Provide basic health education on topics such as nutrition, exercise, chronic disease management, and mental health.
- Document interactions and maintain accurate records in accordance with privacy regulations and organisational policies.
- Advocate on behalf of clients to ensure they receive appropriate and timely healthcare services.
- Collaborate with healthcare professionals, social workers, and other community organisations to coordinate care.
- Participate in team meetings, training sessions, and professional development opportunities.
- Identify and report any barriers to health access or emerging health concerns within the community.
Required Skills
- High School Certificate or equivalent
- Excellent verbal and written communication skills
- Strong interpersonal skills with a compassionate and empathetic approach
- Ability to work effectively with diverse populations and cultural backgrounds
- Basic computer literacy (Microsoft Office Suite, email)
- Strong organisational and time management skills
- Ability to work independently and as part of a team

Preferred Qualifications
- Certificate III or IV in Community Services, Health Support Services, or a related field
- Experience volunteering or working in a community-based setting
