Key Responsibilities

  • Serve as the primary point of contact for clients, their families, and healthcare professionals.
  • Assess individual client needs and develop comprehensive, person-centred support plans.
  • Coordinate appointments, transport, and various healthcare services, ensuring timely and efficient delivery.
  • Maintain accurate and confidential client records, adhering to all data protection regulations.
  • Liaise effectively with internal teams and external agencies to ensure continuity of care.
  • Monitor client progress, identify potential issues, and adapt support plans as necessary.
  • Provide emotional support and guidance to clients and their families with empathy and discretion.
  • Advocate for client rights and preferences within the healthcare system.
  • Assist with administrative tasks related to client care and service delivery.
  • Participate in team meetings and training sessions to continuously improve service quality.

Required Skills

  • Minimum of 18 months experience in a healthcare, social care, or administrative support role.
  • Excellent communication and interpersonal skills, both written and verbal.
  • Strong organisational abilities and attention to detail.
  • Proficiency in using office software (e.g., Microsoft Office Suite, client management systems).
  • Ability to work independently and as part of a multidisciplinary team.
  • Demonstrated empathy, patience, and a non-judgmental approach.
  • Problem-solving skills and ability to adapt to changing priorities.
  • Understanding of confidentiality and data protection principles (e.g., GDPR).

Preferred Qualifications

  • SVQ Level 3 in Health and Social Care or equivalent qualification.
  • Experience working with vulnerable individuals or diverse client groups.
  • Knowledge of local healthcare services and community resources in Edinburgh.
  • Full UK driving license and access to a vehicle.
