Key Responsibilities
- Provide direct patient care, including assisting with personal hygiene, mobility, feeding, and other activities of daily living.
- Monitor and record vital signs, document patient observations, and report any changes in patient condition to nursing staff.
- Assist nurses with medical procedures, treatments, and examinations as directed and within the scope of practice.
- Maintain a clean, safe, and organized patient environment, adhering strictly to infection control protocols.
- Offer specialized support to new patients and their families undergoing relocation, providing information on local amenities, transportation, and community resources.
- Act as a compassionate point of contact for relocating families, addressing non-medical concerns and helping them adjust to the Jacksonville area.
- Assist with patient admissions, discharges, and transfers, ensuring comfort and accurate documentation.
- Promote patient independence and dignity while ensuring their comfort and well-being.
- Participate in care planning meetings and contribute to the development of individualized care plans.
- Ensure compliance with all facility policies, procedures, and regulatory requirements.
Required Skills
- Certified Nursing Assistant (CNA) certification or equivalent.
- Minimum 1 year of experience in a healthcare assistant or patient care role.
- Exceptional interpersonal and communication skills, both written and verbal.
- Strong empathy, patience, and ability to build rapport with patients and families.
- Proficiency in basic patient care techniques and vital sign monitoring.
- Ability to maintain confidentiality and adhere to HIPAA regulations.
- Strong organizational skills and attention to detail.

Preferred Qualifications
