As a Home Health Nurse, you will be the cornerstone of patient well-being, delivering a wide range of services from medication management and wound care to chronic disease management and palliative support. Key Responsibilities

  • Conduct comprehensive in-home patient assessments, including physical and psychosocial status.
  • Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and physicians.
  • Administer medications, perform wound care, and manage various medical equipment (e.g., IVs, catheters).
  • Educate patients and caregivers on disease management, medication adherence, and health promotion strategies.
  • Monitor and document patient progress, changes in condition, and response to treatment.
  • Coordinate care with other healthcare professionals, specialists, and community resources.
  • Provide palliative care and end-of-life support with sensitivity and compassion.
  • Maintain accurate and timely clinical records in compliance with professional standards and organizational policies.
  • Adhere to all safety protocols and infection control procedures.

Required Skills

  • Registered Nurse (RN) license in Nova Scotia, Canada, in good standing
  • Minimum 2 years of nursing experience, preferably in home health, community health, or acute care settings.
  • Current Basic Life Support (BLS) certification.
  • Proficiency in clinical assessment, wound care, and medication administration.
  • Excellent interpersonal and communication skills, both written and verbal.
  • Ability to work independently and manage a caseload effectively.
  • Valid driver's license and access to a reliable vehicle for patient visits.
  • Strong problem-solving and critical thinking abilities.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Experience with electronic health records (EHR) systems specific to home care.
  • Certification in specific areas such as Gerontology, Palliative Care, or Chronic Disease Management.
  • Fluency in additional languages pertinent to the local community.
