Job Description
Career.zycto is seeking a compassionate and dedicated Full-Time Medical Social Worker to join our dynamic healthcare team in Bedford, Halifax. This pivotal role involves providing comprehensive psychosocial support and intervention to patients and their families navigating various health challenges, from acute illnesses to chronic conditions and end-of-life care. As a Medical Social Worker, you will be instrumental in assessing the social, emotional, environmental, and financial factors impacting patient health and recovery, developing tailored care plans that address their unique needs.
You will collaborate closely with an interdisciplinary team, including physicians, nurses, therapists, and other healthcare professionals, to ensure a holistic approach to patient care. Your expertise will be crucial in facilitating effective communication between patients, families, and healthcare providers, ensuring that patient preferences and values are respected and integrated into treatment decisions. Key Responsibilities
- Conduct comprehensive psychosocial assessments of patients and their families to identify needs and strengths.
- Develop, implement, and monitor individualized care plans, including discharge planning and post-hospital care coordination.
- Provide crisis intervention and supportive counseling to patients and families coping with illness, trauma, grief, and loss.
- Advocate for patients' rights and needs, ensuring access to appropriate healthcare and community resources.
- Facilitate communication and collaboration among patients, families, and interdisciplinary healthcare teams.
- Educate patients and families on available resources, support groups, and financial assistance programs.
- Maintain accurate and timely documentation of all patient interactions, assessments, and interventions in accordance with professional and organizational standards.
- Participate in team meetings, case conferences, and professional development activities.
- Identify and address ethical dilemmas, ensuring patient autonomy and well-being are prioritized.
- Contribute to program development and quality improvement initiatives within the social work department.
Required Skills
- Master's degree in Social Work (MSW) from an accredited university.
- Current registration and in good standing with the Nova Scotia College of Social Workers (NSCSW).
- Minimum of 3 years of experience as a Medical Social Worker in a healthcare setting (hospital, long-term care, or community health).
- Demonstrated ability to perform psychosocial assessments and develop effective care plans.
- Proficiency in crisis intervention and supportive counseling techniques.
- Strong knowledge of community resources and healthcare systems in Nova Scotia.
- Excellent written and verbal communication skills, with an ability to communicate complex information clearly and empathetically.
- Proven ability to work effectively as part of an interdisciplinary team.
- Exceptional organizational and time management skills, capable of managing a diverse caseload.
- Proficiency in electronic health record (EHR) systems and standard office software.
Preferred Qualifications
- Experience working with diverse patient populations and cultural backgrounds.
- Training or certification in specific therapeutic modalities (e.g., CBT, DBT).
- Knowledge of mental health and addiction services.
- Bilingualism (English/French) is an asset.
