Key Responsibilities

  • Provide direct personal care, emotional support, and comfort to patients during the night shift, adhering strictly to individualized care plans.
  • Monitor patients' vital signs, physical condition, and emotional well-being, accurately documenting observations and reporting any changes promptly to the nursing staff.
  • Administer medication as prescribed and delegated by registered nurses, ensuring all protocols and safety guidelines are meticulously followed.
  • Act as a senior point of contact during the night, offering guidance, support, and mentorship to junior care assistants.
  • Assist patients with mobility, feeding, hygiene, and other daily living activities, always maintaining dignity and promoting independence where possible.
  • Engage with patients and their families, providing compassionate communication and support, particularly during sensitive end-of-life discussions.
  • Maintain a clean, safe, and therapeutic environment, ensuring all equipment is in good working order and supplies are stocked.
  • Complete comprehensive and accurate electronic and paper documentation related to patient care, incidents, and observations.
  • Participate in handover reports and communicate effectively with the day shift team to ensure continuity of care.
  • Adhere to all safeguarding policies and procedures, ensuring the safety and well-being of all patients.

Required Skills

  • Minimum of 2 years of experience in a palliative care, hospice, or elderly care setting.
  • NVQ Level 2 or 3 in Health & Social Care (or equivalent professional qualification).
  • Demonstrable experience in a senior or lead care assistant role.
  • Exceptional communication, interpersonal, and active listening skills.
  • Profound empathy, patience, and a compassionate approach to care.
  • Ability to work autonomously and make sound judgments, particularly during night hours.
  • Strong organizational skills and meticulous attention to detail in record-keeping and care delivery.
  • Understanding of safeguarding principles and best practices in end-of-life care.
  • Proficiency in basic IT skills for electronic care planning and documentation.

Preferred Qualifications

  • NVQ Level 3 in Health & Social Care (if Level 2 is required).
  • Specialized training or certifications in palliative care or end-of-life care.
  • First Aid certification.
  • Experience with medication administration training (e.g., QCF Level 2 or 3 unit).
