Key Responsibilities
- Conduct comprehensive needs assessments for service users to understand their unique requirements and aspirations.
- Develop, implement, and regularly review person-centred support plans in collaboration with service users, families, and relevant stakeholders.
- Coordinate and schedule support staff, ensuring appropriate matching of skills and personalities to individual needs.
- Act as a primary point of contact for service users and their families, providing guidance, information, and emotional support.
- Monitor the quality and effectiveness of support services, making adjustments as necessary to ensure optimal outcomes.
- Maintain accurate and confidential records, ensuring compliance with all regulatory and organisational policies.
- Advocate for service users' rights and preferences, connecting them with community resources and promoting independence.
- Participate in team meetings, training sessions, and continuous professional development initiatives.
- Handle administrative tasks related to service user management and team coordination efficiently.
Required Skills
- Proven experience in a support, care, or coordination role within social care or healthcare.
