Key Responsibilities
- Dispense prescription medications accurately and efficiently, ensuring compliance with all state and federal regulations.
- Provide comprehensive medication therapy management (MTM) and patient counseling regarding drug indications, dosages, side effects, and proper administration.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care plans and resolve medication-related issues.
- Review patient profiles for drug interactions, contraindications, allergies, and appropriate dosing.
- Maintain accurate patient records and manage pharmacy inventory.
- Educate patients on disease states, healthy lifestyle choices, and the proper use of medical devices.
- Participate in quality improvement initiatives and adhere to all pharmacy policies and procedures.
- Oversee pharmacy technicians and support staff, ensuring efficient workflow and adherence to professional standards.
Required Skills
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Current and active Pharmacist license or eligibility for licensure in the state of Connecticut.
- Minimum of 2 years of experience as a practicing Pharmacist.
