Key Responsibilities
- Implement individualised rehabilitation programs under the guidance of therapists and senior staff.
- Assist clients with daily living activities, personal care, and mobility, promoting independence and dignity.
- Encourage and support clients in performing therapeutic exercises and engaging in meaningful activities.
- Monitor and accurately record client progress, observations, and any changes in condition, reporting promptly to the care team.
- Provide emotional support and companionship, fostering a positive, safe, and engaging environment for clients.
- Support clients with community integration, participation in social activities, and accessing local amenities.
- Maintain accurate and up-to-date client records, documentation, and care plans.
- Adhere strictly to all health and safety policies, safeguarding procedures, and professional boundaries.
- Communicate effectively and empathetically with clients, their families, and multidisciplinary team members.
