Key Responsibilities

  • Conduct comprehensive telephonic assessments of member health status, needs, and goals.
  • Develop, implement, and monitor individualized care plans in collaboration with members, their families, and healthcare providers.
  • Coordinate access to necessary medical, social, and community resources to support care plan objectives.
  • Act as a patient advocate, ensuring members understand their options and rights within the healthcare system.
  • Facilitate communication and collaboration among all parties involved in a member's care.
  • Educate members on health conditions, self-management strategies, and preventive care.
  • Maintain accurate and timely documentation of all case management activities and interventions using electronic health records.
  • Identify and address potential barriers to care, working proactively to find solutions.

Required Skills

  • Excellent verbal and written communication skills with a strong ability to build rapport remotely.
  • Proven organizational and time management skills, capable of managing a diverse caseload effectively.
  • Strong problem-solving and critical-thinking abilities to navigate complex patient situations.
  • Proficiency in using digital tools, telehealth platforms, and electronic health records (EHR).
  • Empathy, compassion, and a patient-centred approach to care.
  • Ability to work independently and as part of a virtual team with minimal supervision.
  • In-depth understanding of the UK healthcare system and available resources.
  • Clinical background (e.g., Nurse, Social Worker, Allied Health Professional) with relevant registration/licensure in the UK.

  • Bachelor’s degree in Nursing, Social Work, or a related healthcare field.
  • Certification in Case Management (e.g., CCM or similar).
  • Experience working with diverse patient populations and complex chronic conditions.
