Key Responsibilities

  • Lead the assessment, planning, implementation, and review of person-centred care packages.
  • Manage a caseload of complex care clients, ensuring high-quality, continuous support.
  • Conduct initial client assessments and regular reviews to ensure care plans meet evolving needs.
  • Supervise, mentor, and provide guidance to junior care coordinators and support staff.
  • Liaise effectively with clients, families, healthcare professionals, and external agencies.
  • Ensure all care services comply with CQC regulations, company policies, and best practices.
  • Address and resolve complex care queries and complaints with sensitivity and professionalism.
  • Participate in on-call rota duties as required to provide senior support.
  • Maintain accurate and up-to-date client records and care documentation.
  • Identify opportunities for service improvement and contribute to policy development.

Required Skills

  • Minimum 4 years' experience in a care coordination or senior care role.
  • In-depth knowledge of social care legislation, CQC standards, and safeguarding procedures.
  • Proven ability to develop, implement, and manage complex care plans.
  • Exceptional communication and interpersonal skills, both written and verbal.
  • Strong organizational and time management abilities, with attention to detail.
  • Proficiency in using care management software and general office applications.
  • Full UK driving license and access to a reliable vehicle.
  • NVQ Level 3 in Health & Social Care (or equivalent).

Preferred Qualifications

  • NVQ Level 5 Diploma in Leadership for Health and Social Care Services (or working towards).
  • Experience in mental health, learning disabilities, or palliative care settings.
  • Previous experience in supervising or mentoring staff.
  • Familiarity with local health and social care services in York.
