Key Responsibilities
- Lead the assessment, planning, implementation, and review of person-centred care packages.
- Manage a caseload of complex care clients, ensuring high-quality, continuous support.
- Conduct initial client assessments and regular reviews to ensure care plans meet evolving needs.
- Supervise, mentor, and provide guidance to junior care coordinators and support staff.
- Liaise effectively with clients, families, healthcare professionals, and external agencies.
- Ensure all care services comply with CQC regulations, company policies, and best practices.
- Address and resolve complex care queries and complaints with sensitivity and professionalism.
- Participate in on-call rota duties as required to provide senior support.
- Maintain accurate and up-to-date client records and care documentation.
- Identify opportunities for service improvement and contribute to policy development.
Required Skills
- Minimum 4 years' experience in a care coordination or senior care role.
- In-depth knowledge of social care legislation, CQC standards, and safeguarding procedures.
- Proven ability to develop, implement, and manage complex care plans.
- Exceptional communication and interpersonal skills, both written and verbal.
- Strong organizational and time management abilities, with attention to detail.
- Proficiency in using care management software and general office applications.
- Full UK driving license and access to a reliable vehicle.
- NVQ Level 3 in Health & Social Care (or equivalent).
Preferred Qualifications
- NVQ Level 5 Diploma in Leadership for Health and Social Care Services (or working towards).
- Experience in mental health, learning disabilities, or palliative care settings.
- Previous experience in supervising or mentoring staff.
- Familiarity with local health and social care services in York.
