Job Description

We are seeking a dedicated and compassionate Social Work Case Coordinator to join our dynamic team in Selibe-Phikwe, Central District. This pivotal role involves managing and coordinating a diverse caseload of individuals and families requiring social support and intervention. The successful candidate will be responsible for conducting comprehensive psychosocial assessments, developing individualized care plans, and facilitating seamless access to a wide range of community resources and services. Key Responsibilities

  • Conduct thorough psychosocial assessments of individuals and families to accurately identify their needs, strengths, challenges, and aspirations.
  • Develop, implement, and meticulously monitor individualized care plans in close collaboration with clients, their families, and all relevant stakeholders.
  • Coordinate and facilitate timely access to appropriate internal and external resources, including but not limited to healthcare, educational, legal, and financial assistance programs.
  • Provide ongoing support, empathetic counseling, and practical guidance to clients, empowering them to make informed decisions and progress towards self-sufficiency.
  • Actively advocate for clients' rights and needs within various systemic and institutional frameworks.
  • Maintain accurate, confidential, and meticulously up-to-date client records and case documentation, strictly adhering to professional ethical standards and organizational policies.
  • Participate actively in team meetings, case conferences, and regular supervision sessions to ensure integrated and highly effective service delivery.
  • Collaborate proactively with community leaders, government agencies, and other non-governmental organizations to strengthen referral pathways and enhance community support networks.
  • Contribute significantly to the collection and analysis of program data for precise reporting, thorough evaluation, and impactful advocacy purposes.
  • Identify and promptly report any safeguarding concerns or risks to relevant authorities, diligently following all established protocols and guidelines.
  • Engage in continuous professional development to stay abreast of the latest best practices, innovative approaches, and emerging trends in social work and community development.

Required Skills

  • Bachelor’s Degree in Social Work or a related field (e.g., Psychology, Sociology)
  • Minimum of 2 years of proven experience in case management or social work practice.
  • Demonstrated ability to conduct comprehensive assessments and develop effective intervention plans.
  • Strong understanding of community resources and social welfare systems in Botswana.
  • Excellent communication, interpersonal, and advocacy skills.
  • Proficiency in record-keeping and case documentation.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to ethical social work practice and client confidentiality.
  • Fluency in English and Setswana (both written and spoken).

Preferred Qualifications

  • Master’s Degree in Social Work or related field.
  • Experience working with vulnerable populations (e.g., youth, HIV/AIDS affected, gender-based violence survivors).
  • Familiarity with psychosocial support programs.
  • Valid driver’s license.
