Key Responsibilities

  • Manage and coordinate weekend care schedules, ensuring appropriate staffing levels and continuity of care for all clients.
  • Act as the primary point of contact for care assistants, clients, and their families during weekend hours, addressing queries and resolving issues promptly and effectively.
  • Monitor and respond to emergency calls, coordinating immediate actions and necessary support.
  • Provide administrative support, including accurate record-keeping of care plans, rota changes, and communications.
  • Support care assistants with queries, guidance, and problem-solving, fostering a positive and collaborative team environment.
  • Ensure compliance with all company policies, procedures, and relevant care regulations (e.g., SSSC, Care Inspectorate Scotland).
  • Participate in on-call duties as required, providing out-of-hours support.
  • Assist with new client assessments and care plan reviews as necessary.
  • Report any safeguarding concerns or incidents immediately to the appropriate management.
  • Contribute to continuous improvement initiatives within the care coordination team.

Required Skills

  • Proven experience in care coordination or a similar role within the social care sector (minimum 1 year).
  • Excellent communication and interpersonal skills, with the ability to liaise effectively with diverse groups.
  • Strong organisational and time management abilities, capable of managing multiple priorities under pressure.
  • Proficiency in using scheduling software and Microsoft Office Suite (Word, Excel, Outlook).
  • Exceptional problem-solving skills and the ability to make sound decisions independently.
  • A compassionate and empathetic approach to client and staff needs.
  • Full UK Driving License and access to a reliable vehicle.
  • Understanding of care regulations and best practices (e.g., SSSC Codes of Practice, Care Inspectorate Scotland requirements).

Preferred Qualifications

  • SVQ Level 3 in Health and Social Care or equivalent qualification.
  • Experience with care management systems (e.g., CM2000, People Planner, CareFree).
  • Prior experience working specifically with elderly or vulnerable adults.
  • Familiarity with the Abbeyhill/Edinburgh area.
