Key Responsibilities
- Conduct virtual patient assessments, triage calls, and provide medical advice based on established protocols.
- Educate patients and their families on health conditions, medications, treatment plans, and preventative care.
- Monitor and evaluate patient responses to care, making adjustments to care plans as necessary.
- Collaborate effectively with physicians, specialists, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate, detailed, and timely electronic health records (EHR) of all patient interactions.
- Provide guidance and support for common acute and chronic conditions, promoting self-management strategies.
- Adhere strictly to all professional nursing standards, ethical guidelines, and legal requirements.
- Participate in ongoing training and professional development to stay current with telehealth best practices and clinical advancements.
- Handle patient inquiries and concerns with empathy and professionalism, ensuring high patient satisfaction.
Required Skills
- Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO)
- Minimum of 3 years of clinical nursing experience in acute care, primary care, or a similar setting
- Proficiency with electronic health records (EHR) systems and telehealth platforms
- Excellent verbal and written communication skills with strong active listening abilities
- Demonstrated ability to perform comprehensive patient assessments and critical thinking in a remote setting
- Strong organizational skills and ability to manage multiple patient cases simultaneously
- Reliable high-speed internet connection and a dedicated, private home office space
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Previous experience in telehealth or remote patient care
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS)
- Familiarity with medical terminology in various specialties
- Experience working autonomously and managing a diverse patient caseload
