Job Description

Key Responsibilities

Required Skills
- Proven experience in a support worker, social care, or community outreach role.
- Excellent interpersonal and communication skills, both written and verbal.

- Proficiency in case management and record-keeping.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Knowledge of local government services and community resources in London.
Preferred Qualifications
- NVQ Level 3 or equivalent in Health & Social Care or a related field.
- Experience working with vulnerable adults or families in an urban setting.
- Knowledge of safeguarding policies and procedures.
- Full UK driving license.
