Key Responsibilities
- Conduct comprehensive assessments of patient needs, including medical, social, psychological, and environmental factors.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare providers.
- Coordinate and facilitate access to a wide range of services, including medical appointments, specialist referrals, social support, and community resources.
- Educate patients and their families on health conditions, treatment options, medication management, and self-care strategies.
- Act as a central point of contact, ensuring clear and consistent communication among all stakeholders involved in a patient's care.
- Advocate for patients' rights and preferences, ensuring their voices are heard and respected in care decisions.
- Maintain accurate and confidential patient records, documenting all interactions, assessments, and care plan progress.
- Identify barriers to care and work proactively to resolve them, adapting care plans as patient needs evolve.
- Participate in team meetings, case conferences, and professional development opportunities to enhance care coordination skills.
Required Skills
- Exceptional verbal and written communication skills in Portuguese
- Strong organizational and time management abilities
- Demonstrated empathy, compassion, and active listening skills
- Proficiency in basic computer applications and electronic health records (EHR) systems
- Ability to work effectively both independently and as part of a multidisciplinary team
- Problem-solving aptitude with the ability to navigate complex situations

Preferred Qualifications

- Familiarity with chronic disease management principles and preventive care strategies
- Fluency in additional local languages relevant to Nampula (e.g., Macua, Lomwe)
- Certification in Case Management or Care Coordination
