Key Responsibilities

  • Conduct comprehensive assessments of patient needs, including medical, social, psychological, and environmental factors.
  • Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare providers.
  • Coordinate and facilitate access to a wide range of services, including medical appointments, specialist referrals, social support, and community resources.
  • Educate patients and their families on health conditions, treatment options, medication management, and self-care strategies.
  • Act as a central point of contact, ensuring clear and consistent communication among all stakeholders involved in a patient's care.
  • Advocate for patients' rights and preferences, ensuring their voices are heard and respected in care decisions.
  • Maintain accurate and confidential patient records, documenting all interactions, assessments, and care plan progress.
  • Identify barriers to care and work proactively to resolve them, adapting care plans as patient needs evolve.
  • Participate in team meetings, case conferences, and professional development opportunities to enhance care coordination skills.

Required Skills

  • Exceptional verbal and written communication skills in Portuguese
  • Strong organizational and time management abilities
  • Demonstrated empathy, compassion, and active listening skills
  • Proficiency in basic computer applications and electronic health records (EHR) systems
  • Ability to work effectively both independently and as part of a multidisciplinary team
  • Problem-solving aptitude with the ability to navigate complex situations
  • Knowledge of local health resources and social support systems in Nampula Province

Preferred Qualifications

  • Bachelor's degree in Nursing, Social Work, Public Health, or a related field
  • Previous experience (3+ years) in a care coordination, case management, or patient navigation role
  • Familiarity with chronic disease management principles and preventive care strategies
  • Fluency in additional local languages relevant to Nampula (e.g., Macua, Lomwe)
  • Certification in Case Management or Care Coordination
