Job Description

We are urgently seeking a dedicated and compassionate Case Manager to join our dynamic team in Milange, Zambezia Province, with an immediate start requirement. This critical role involves working directly with individuals and families facing various challenges, providing essential support, advocacy, and connection to vital community resources. As a Case Manager, you will be the cornerstone of support for our clients, guiding them through complex systems, developing personalized care plans, and monitoring their progress to ensure sustainable positive outcomes.

This position demands a proactive individual with excellent communication and organizational skills, capable of managing a diverse caseload while maintaining a client-centered approach. Key Responsibilities

  • Conduct comprehensive intake assessments to determine client needs and eligibility for services.
  • Develop, implement, and monitor individualized service plans in collaboration with clients.
  • Provide ongoing case management, including advocacy, support, and crisis intervention.
  • Connect clients to appropriate internal and external resources, including healthcare, housing, education, and social services.
  • Maintain accurate and confidential client records, documenting all interactions and service provisions.
  • Monitor client progress towards goals and adjust service plans as necessary.
  • Advocate for clients' rights and access to services within various systems.
  • Collaborate with community partners, government agencies, and other stakeholders.
  • Facilitate group sessions or workshops on life skills, wellness, or other relevant topics.
  • Adhere to ethical standards, organizational policies, and relevant legal regulations.

Required Skills

  • Proven experience in case management or a related social services role
  • Strong understanding of client-centered service delivery
  • Excellent interpersonal and communication skills (written and verbal)
  • Ability to work independently and as part of a multidisciplinary team
  • Exceptional organizational and time management abilities
  • Proficiency in basic computer applications (e.g., Microsoft Office Suite)
  • Cultural sensitivity and ability to work with diverse populations
  • Strong problem-solving and critical thinking skills
  • Fluency in Portuguese (essential for local communication)
  • Valid driver's license and willingness to travel locally within Milange/Zambezia

Preferred Qualifications

  • Bachelor's degree in Social Work, Psychology, Public Health, or a related field
  • Experience working within the Milange, Zambezia Province community
  • Knowledge of local government and non-profit services in Mozambique
  • Proficiency in local languages spoken in Zambezia Province
