In this vital and engaging role, you will work closely under the supervision of Registered Nurses and other healthcare professionals to meticulously implement individualized care plans. Your daily responsibilities will be diverse and impactful, involving assisting patients with a wide range of activities of daily living (ADLs), which include personal hygiene, bathing, dressing, grooming, feeding, safe ambulation, and transfer assistance. You will play a critical role in observing and accurately reporting any changes in patient condition, meticulously documenting the care provided, and diligently maintaining a clean, safe, and organized patient environment. Key Responsibilities

  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
  • Provide assistance with mobility, transfers, and ambulation using proper body mechanics and assistive devices.
  • Monitor and accurately record vital signs, intake and output, and other relevant patient data.
  • Observe and promptly report any changes in patient physical, mental, or emotional condition to nursing staff.
  • Maintain a clean, safe, and organized patient environment, including changing linens and tidying patient rooms.
  • Support patient independence and dignity while providing compassionate and respectful care.
  • Assist with personal care, hygiene, and comfort measures.
  • Document care provided thoroughly and accurately in patient records.
  • Communicate effectively and professionally with patients, families, and the healthcare team.
  • Adhere to all facility policies, procedures, and safety regulations.

Required Skills

  • Valid Certified Nursing Assistant (CNA) certification in California.
  • Current Basic Life Support (BLS) certification.
  • Minimum of 1 year of experience as a CNA in a healthcare setting (e.g., long-term care, acute care, skilled nursing).
  • Excellent communication and interpersonal skills.
  • Strong observational and reporting abilities.
  • Ability to work effectively as part of a multidisciplinary team.
  • Compassionate, patient-centered approach to care.
  • Physical ability to perform duties, including lifting, bending, and standing for extended periods.
  • Reliable and punctual with a strong work ethic.

Preferred Qualifications

  • Experience with electronic health records (EHR) systems.
  • Experience in a post-acute rehabilitation or skilled nursing facility.
  • Fluency in Spanish or other languages common in the community.
