Job Description

Career.zycto is seeking a dedicated and compassionate Geriatric Care Manager to join our growing team in the heart of Harbourfront, Toronto. This full-time position offers a unique opportunity to play a pivotal role in enhancing the quality of life for older adults and their families. As a Geriatric Care Manager, you will serve as a vital advocate and navigator, providing comprehensive support, guidance, and coordination for seniors facing complex health, social, and emotional challenges. Your work will directly contribute to empowering individuals to maintain their independence, improve their well-being, and access the best possible resources within their community.

In this multifaceted role, you will be responsible for conducting thorough assessments, developing personalized care plans, and implementing strategies that address a wide spectrum of needs, from medical and cognitive to social and financial. You will work collaboratively with clients, their families, healthcare providers, and community organizations to ensure seamless transitions and continuous, high-quality care. This includes coordinating appointments, managing medications, arranging for home care services, identifying suitable housing options, and facilitating access to recreational and therapeutic programs. Key Responsibilities

  • Conduct comprehensive initial and ongoing assessments of client needs, including physical, cognitive, psychological, social, and environmental factors.
  • Develop, implement, and monitor individualized care plans that address identified needs, goals, and preferences of clients and their families.
  • Coordinate and manage a wide range of services, including medical appointments, home care, rehabilitation, transportation, and community programs.
  • Act as a primary liaison and advocate for clients with healthcare providers, family members, legal/financial professionals, and other service agencies.
  • Provide ongoing support, education, and guidance to clients and their families, assisting them in navigating the healthcare system and making informed decisions.
  • Identify and facilitate access to appropriate local resources, government programs, and financial assistance options for seniors.
  • Monitor client progress, respond to emergent situations, and adjust care plans as necessary to ensure optimal outcomes and safety.
  • Maintain accurate, confidential, and up-to-date client records and documentation in compliance with organizational policies and regulatory standards.
  • Facilitate communication and resolve conflicts between family members, ensuring a cohesive approach to care.

Required Skills

  • Bachelor's degree in Social Work, Nursing, Gerontology, Psychology, or a related healthcare field.
  • Minimum of 4 years of experience in geriatric care management, case management, or a similar role working with seniors.
  • Strong understanding of the aging process, common geriatric conditions, and relevant healthcare systems and resources in Ontario.
  • Exceptional interpersonal and communication skills (verbal and written) with the ability to build rapport and trust with clients and families.
  • Proven ability to assess complex situations, develop effective care plans, and manage multiple cases simultaneously.
  • Proficiency in crisis intervention, problem-solving, and conflict resolution.
  • Demonstrated empathy, patience, and cultural sensitivity when working with diverse populations.
  • Strong organizational skills and attention to detail for accurate record-keeping and service coordination.
  • Valid Ontario driver’s license and access to a reliable vehicle for client visits across the Harbourfront area and potentially wider Toronto.

Preferred Qualifications

  • Master's degree in Social Work, Nursing, Public Health, or a related discipline.
  • Certification as a Certified Case Manager (CCM) or Certified Geriatric Care Manager (CGCM).
  • Experience with electronic health records (EHR) systems.
  • Fluency in additional languages relevant to Toronto's diverse population (e.g., Mandarin, Cantonese, Italian, Tagalog).
  • Knowledge of local community services and support networks specific to the Harbourfront and greater Toronto area.
