Key Responsibilities
- Provide compassionate personal care including bathing, dressing, grooming, and oral hygiene, maintaining client dignity at all times.
- Assist clients with mobility, transfers, and positioning to ensure comfort and prevent pressure injuries.
- Monitor and accurately record vital signs, intake/output, and changes in client's physical or emotional condition, reporting observations to the nursing team.
- Offer emotional support, companionship, and active listening to clients and their families, fostering a comforting presence.
- Assist with light meal preparation, feeding, and hydration as required, adhering to dietary restrictions and preferences.
- Maintain a clean, safe, and organized environment within the client's living space.
- Facilitate activities that promote cognitive and emotional well-being, as outlined in the care plan.
- Adhere strictly to all care plans, policies, and procedures, ensuring client safety and quality of care.
- Provide medication reminders, ensuring clients take their prescribed medications on schedule (not administering).
- Collaborate effectively with interdisciplinary team members, including nurses, doctors, and other healthcare professionals.
Required Skills
- Valid Palliative Care Aide certification, Home Support Worker certificate, or equivalent.
- Demonstrated profound empathy, compassion, and sensitivity towards individuals with life-limiting illnesses.
- Exceptional interpersonal and communication skills, both verbal and written.
- Ability to work independently with minimal supervision and as an integral part of a care team.
- Proven reliability, punctuality, and a strong work ethic.
- Physical ability to perform duties, including lifting, bending, and standing for extended periods.
- Basic computer literacy for documentation and communication.
- Cultural sensitivity and respect for diverse backgrounds and beliefs.
Preferred Qualifications
- Minimum of 1 year of experience in a hospice, palliative care, or long-term care setting.
- Current First Aid and CPR certification.
- Knowledge of common palliative care approaches and pain management techniques.
- Fluency in additional languages is considered an asset.
