Job Description

We are seeking a dedicated and compassionate Palliative Care Nurse to join our exceptional team in Dundas, Hamilton. Key Responsibilities

  • Assess, plan, implement, and evaluate nursing care for patients with life-limiting illnesses.
  • Provide expert symptom management, including pain control, nausea, fatigue, and other distressing symptoms.
  • Offer emotional, psychological, and spiritual support to patients and their families.
  • Educate patients and families on disease progression, treatment options, and comfort measures.
  • Collaborate effectively with an interdisciplinary team (physicians, social workers, spiritual care, etc.) to ensure comprehensive care.
  • Facilitate advanced care planning discussions and uphold patient wishes and values.
  • Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
  • Document patient assessments, interventions, and outcomes accurately and thoroughly.
  • Participate in care conferences and contribute to continuous quality improvement initiatives.
  • Provide compassionate end-of-life care, focusing on comfort and dignity.

Required Skills

  • Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO).
  • Minimum 2 years of recent experience in palliative care, oncology, hospice, or a related field.
  • Demonstrated expertise in symptom management and pain assessment.
  • Exceptional communication, interpersonal, and active listening skills.
  • Strong empathy, compassion, and sensitivity to the needs of patients and families facing life-limiting illnesses.
  • Ability to work effectively both independently and as part of an interdisciplinary team.
  • Proficiency in electronic health record (EHR) systems.
  • Valid Basic Life Support (BLS) certification.

Preferred Qualifications

  • Certification in Hospice and Palliative Care (CHPCN or equivalent).
  • Advanced education in palliative care or related specialty.
  • Experience with grief and bereavement support.
  • Fluency in additional languages.
