As a Social Services Case Manager, you will manage a diverse caseload, conducting thorough assessments to understand client needs and strengths. You will then develop individualized service plans, connecting clients with essential resources such as housing assistance, healthcare, employment services, mental health support, and educational opportunities. This role demands a proactive approach to monitoring client progress, advocating on their behalf, and adapting support strategies as circumstances evolve. Key Responsibilities
- Conduct comprehensive intake assessments to determine client needs, strengths, and eligibility for services.
- Develop, implement, and monitor individualized service plans in collaboration with clients, setting measurable goals.
- Connect clients with appropriate community resources, including housing, healthcare, employment, education, and legal aid.
- Provide ongoing case management, crisis intervention, advocacy, and emotional support to clients.
- Maintain accurate, timely, and confidential client records and documentation in compliance with organizational standards and regulations.
- Collaborate effectively with internal teams, external agencies, and community partners to ensure coordinated care.
- Monitor client progress towards goals, reassess needs periodically, and adjust service plans as necessary.
- Educate clients on their rights, available resources, and strategies for self-advocacy.
- Participate in team meetings, training sessions, and professional development opportunities.
- Ensure compliance with all federal, state, and local regulations pertaining to social services.
Required Skills
- Bachelor's degree in Social Work, Psychology, Sociology, or a related human services field.
- Minimum of 3 years of direct case management experience in social services.
- Strong understanding of social determinants of health and systemic barriers.
- Excellent verbal and written communication skills, with ability to build rapport.
- Proficiency in crisis intervention and de-escalation techniques.
- Demonstrated ability to manage a diverse caseload effectively and prioritize tasks.
- Strong organizational skills and attention to detail for record-keeping.
- Proficiency with Microsoft Office Suite and client management software.
- Ability to work independently and as part of a collaborative team.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Master's degree in Social Work (MSW) or a related field.
- LCSW or relevant licensure/certification.
- Bilingual proficiency (especially Spanish) is a significant asset.
- Experience working with diverse populations in urban settings.
- Knowledge of local community resources and social service agencies in Louisville, KY.
