Key Responsibilities
- Provide person-centred emotional, social, and practical support to individuals living with mental health conditions.
- Assist service users with daily living activities, including personal care, meal preparation, and household management, as outlined in their care plans.
- Support service users in developing and maintaining independent living skills and accessing community resources.
- Accompany individuals to appointments, social activities, and community events to promote engagement and reduce isolation.
- Implement and contribute to the review of individual care and support plans, ensuring they reflect current needs and goals.
- Maintain accurate and up-to-date records, adhering to confidentiality and data protection policies.
- Work collaboratively with a multidisciplinary team, families, and external agencies to ensure holistic support.
- Identify and report any safeguarding concerns, ensuring the safety and well-being of service users.
Required Skills
- Minimum 6 months experience in a support worker or care role, preferably within mental health.
