Job Description
We are seeking dedicated and empathetic Support Workers to join our expanding team, serving individuals within the vibrant community of Cardiff Bay, Cardiff. Key Responsibilities
- Provide person-centred care and support to individuals, adhering to their care plans and promoting independence.
- Assist with daily living activities, including personal care, meal preparation, medication management (where applicable), and household tasks.
- Support individuals in accessing community activities, appointments, and social engagements.
- Maintain accurate and up-to-date records of care, observations, and incidents, ensuring confidentiality.
- Communicate effectively with individuals, their families, and other professionals, providing regular updates on progress and concerns.
- Adhere to all safeguarding policies and procedures, ensuring the safety and well-being of the individuals supported.
- Work collaboratively as part of a dedicated team, contributing to a positive and supportive environment.
